1. Building itself looks very old (like it's ready to collapse) - a lot of outdated equipments...
2. No staff teaching done to a patient that stayed for 4 days - had to print out our own teaching materials to give to my elderly grandparents...
3. No fall prevention program in place - pt was identified w/ poor safety awareness due to stroke, no bed alarms in bed, call light not answered promptly (if answered at all) !!!
4. Staff @ 4th floor with VERY POOR customer service - ignored call light while staff are chitchatting @ nursing station; we had to actually walk over to the nsg station but our presence was NOT acknowledged at all; and when we finally got their attention, we asked for the nurse assigned to our pt and the staff just said..."i think she's on break" - DID NOT even bother ask what we needed. I suggest they take "customer service improvement" intensive classes, AIDET training, anybody heard of that?
5. Mixing of infectious, non-infectious, and surgical pts in a room... BIG INFECTION CONTROL issue.
6. Heparin bag was left infusing for 42 hours !!! What are the possible side effects??? then the 2nd Heparin bag was not changed until the family asked the staff to do so...28 hours later! Part of cost-cutting???
7. Ultimately, nurse discharging the patient (Hazel) giving the WRONG information regarding dosage of a highly potent medication (COUMADIN)...MD order was 4 mg, instructing pt's family to give 3 mg --- SAFETY ISSUE???
8. And, prior to D/C, pt got the pneumovax......SUBCUTANEOUSLY !!! Was given by a new nurse (Khan) w/ needle half-way out of the pt's arm !!!
Talk about pt satisfaction......Thank God there was Dr. Verbinski who was very accomodating and always giving us updates. Also thanks to Jonhan, RN from critical care - keep up the good customer service.
